Collaboration scores

The instructor had opportunities to observed how the group worked together during class times, but not outside of class. Please provide the following information to give more information for how the group worked together (or didn't):

1. list the members of the group (starting with yourself),
2. select a collaboration rating for each member of your group (including yourself),
3. optionally provide brief comments for your rating.

Fold in half and return to the instructor. Thanks!
